I/We understand that payments are made the first week of classes. I/We understand the payment made today is only a partial payment toward tuition to secure our spot, the remaining amount of tuition is due at the first class. I/We understand that classes are paid for on a monthly basis even if my dancer misses a class during the month. Prices do not change based on attendance. I/We understand that if we decide to stop attending classes there will not be a refund for the current month if in session, any withdrawals must occur at the start of the month in order to avoid payment. *Note: If you are paying with a credit card there will be a 3% service fee added to your payment in order to cover the cost of the machine. We also accept cash and checks.

I/We understand and appreciate that participation in dance constitutes a risk to my/our son/daughter of possible injury, including serious injury. In consideration of permitting my/our son/daughter to participate in dance at Donna Brum Dancers/Redefine, A Movement Company I/We voluntarily and knowingly recognize, accept and assume the risks associated with my/our son's/daughter's participation in classes and dance. Agree to and do hereby release Donna Brum Dancers/Redefine, A Movement Company, its agents and employees from any liability whatsoever for any injury to my/our son/daughter on account of participation in classes, and agree to hold them harmless from any damages, including attorneys' fees, due to any injury sustained by participation in classes.
